Job Opportunity: OP/LTC LMSW (Medical Social Worker)

Location: Seattle, WA

Job Duration: 13 Weeks

Pay Range: $52.00 to $55.00 on W2 Hourly

About Us: Join our dedicated interdisciplinary team as an OP/LTC Licensed Master Social Worker (LMSW) in Seattle, Washington. We are committed to providing exceptional care to our diverse geriatric patient population across outpatient and long-term care settings.

Responsibilities:

• Conduct comprehensive social work assessments and interventions, focusing on cultural competence and adherence to NASW Code of Ethics.
• Develop personalized care plans in collaboration with our healthcare team.
• Coordinate community-based programs and resources to support seamless patient care.
• Provide crisis intervention and manage high-risk situations such as suicide and self-harm.
• Facilitate discharge planning and coordinate home care services using EPIC electronic health records.

Requirements:

• Minimum 1 year of geriatric social work experience; 2 years preferred with diverse ethnic groups.
• Active Washington Advanced Social Worker License or equivalent (required upon hire).
• BLS certification.
• Proficiency in cultural issues and high-risk intervention.
• Strong interpersonal skills and ability to thrive in a collaborative healthcare environment.
